That broke the Cardinals' streak of 10 straight wins in conference tournament games. They had won the last three Big East tournament titles.
Terry Rozier (20), Wayne Blackshear (18) and Montrezl Harrell (14) combined to score 52 of Louisville's 60 points. Only two other players scored, each with four points.
Rozier scored at least 20 points for the third time against the Tar Heels this season and totaled 67 points in the three meetings. He had 25 in the first game and 22 in the second.
Harrell added 12 rebounds to his 14 points for his 13th double-double of the season. That leads the ACC, breaking a tie with Devin Thomas of Wake Forest and Rakeem Christmas of Syracuse. Harrell has 25 double-doubles in his career.
Quentin Snider had six assists for the Cardinals, his season high.
North Carolina is now 92-43 all-time in ACC Tournament games, including a 38-17 record at the Greensboro Coliseum.
The Tar Heels improved to 3-4 as the No. 5 seed in the ACC Tournament. North Carolina improved to 2-1 vs. Louisville this season after each team won at home during the regular season. UNC leads the all-time series between the schools, 11-4.
With fifth-seeded UNC's win, the No. 4 seed's all-time edge vs. the No. 5 seed in ACC Tournament quarterfinal games was trimmed to 31-27.
UNC shot 50 percent in the second half, marking the fifth straight game in which the Tar Heels have shot 50 percent or better in the final 20 minutes. UNC shot 15-of-17 from the foul line (.882).
Each team's biggest lead was 10 points on one occasion - Louisville at the 5:18 mark of the first half, and North Carolina's final margin of victory, which came on a pair of Brice Johnson free throws with 22 seconds left to play.
With his game-high 22 points in Thursday's contest, North Carolina junior forward Brice Johnson reached double figures for the third straight game and the 24th time this season. Johnson, who scored 17 points in Wednesday's second-round win over Boston College, has averaged 19.5 points in two 2015 ACC Tournament games thus far.
With 13 points on Thursday, North Carolina junior guard Marcus Paige hit double figures for the third consecutive game and team-leading 25th time this season. Paige connected on three shots from 3-point range, giving him 79 for the season and 210 for his career, one behind fifth-place Jeff Lebo on UNC's all-time list.
North Carolina freshman Justin Jackson had 10 points against the Cardinals, giving him seven straight double-figure scoring games and 18 for the season.
North Carolina head coach Roy Williams recorded his 747th career win to rank 15th on the all-time NCAA Division I list.
Williams improved to 17-9 as a head coach in ACC Tournament games. Fourth-seeded Louisville is 24-8 after losing its first-ever ACC Tournament game.
