Dear Men's Basketball Ticket Holder,
We would first like to thank you for your continued support of Cardinal athletics. We are privileged to have you as part of our team!
The NCAA requires all Division I institutions to report a financial disclosure form for their men's basketball program. This form identifies the relationship existing between the institution and any individual or entity involved in the coaching, counseling, advising, and/or sponsoring of men's basketball prospects and/or non-scholastic traveling teams (i.e., AAU or club team). A men's basketball prospect is defined as a student who has started classes for the ninth grade.
On the enclosed information sheet, please answer each question. If any of the answers are "yes", please mail, fax, or email us your responses. If you answer "no" to all the questions, you do not need to respond to us. Even if you completed this form last year, we need you to do it again this year if you have any "yes" responses. It is important for us to obtain all existing relationships you or your company has with prospects in order to comply with NCAA constitution 6.5. Any information we do not report will be in violation of the NCAA bylaw and will result in the denial of all recruiting privileges this summer for our men's basketball program.
